Outreach Worker

Provides outreach services and coordinates client intake for Health Care for the Homeless (HCH) programs; refers clients to appointments with health and social services providers; advocates for clients within the service delivery system; provides education, guidance and follow-up to homeless persons; provides documentation of encounters and referrals; assists in data collection for program activities; participates in staff meetings and case reviews; maintains and updates community resource information; provides reports on outreach issues; identifies challenges and generates options for addressing client issues and problems; responds to request for information; performs other duties as assigned.
KNOWLEDGE, SKILLS AND ABILITIES: Knowledge of Health Department and cooperative Health Center policies and procedures principles and practices of public health; principles of good health habits, nutrition and hygiene; applicable public health statutes and ordinances; functions of other health and social agencies; community health and social service resources. Ability to Conduct client interviews; facilitate problem resolution and crisis intervention; maintain confidentiality; use common office machines; operate personal computers and related software; communicate orally and in writing; establish effective working relationships with fellow employees, supervisors, and citizens. PHYSICAL/ENVIRONMENTAL DEMANDS: Duties are generally performed in an office environment where hazards and discomforts are controlled and modifiable This position requires the ability to stand, bend at the waist, kneel, reach over the head, talk, hear, and see. Must be able to move or lift documents and materials weighing up to 20 pounds. Position requires knowledge and use of typical office equipment including telephone, copiers, fax machines and personal computer. Position requires frequent contact with fellow employees, service agencies and citizens. This position may be assigned additional duties or tasks as part of a response to a public health emergency. QUALIFICATION REQUIREMENTS The above knowledge, skills, and abilities are typically acquired through graduation from high school and direct experience and contact with client population, either in a service role or as a homeless person.
