Full-time Patient Navigator

The Patient Navigator will be the primary point of contact for cancer patients who are treated in healthcare facilities designated as priority sites for the Midwest Division Patient Navigation Program. These sites represent those serving the highest number of newly diagnosed, medically underserved patients. This population is considered in greatest need of Patient Navigation services and is a population that does not otherwise come to the American Cancer Society seeking service. The Patient Navigator will establish contact with patients, survivors and caregivers, through approaches agreed upon with the site(s), to raise awareness about information resources, support programs and services, and assist in meeting other cancer-related needs of patients and caregivers. · Build and maintain relationships with key staff in priority sites to build patient referral mechanisms and promote Patient Navigation service.· Establish relationships and alliances with local service providers and other healthcare providers to enhance service and referral networks in communities. Complete comprehensive assessments of the barriers to quality cancer care that contribute to disparities and identify information, resource and emotional support needs of newly diagnosed and medically underserved patients, survivors, and caregivers. Facilitate constituent access to available ACS and non-ACS services and programs. Establish relationships with constituents seeking information and service based on Division standards. Maintain accurate, confidential records documenting services provided and unmet needs using the Constituent Relationship Database (Siebel) provided. Remain current with resource and referral information found in the Cancer Resource Connection and contribute to the enhancement of the database. Implement a follow-up plan for each constituent to ensure that services have met their needs. Recruit and manage volunteer relationships to support program of work. Undertake related duties as assigned Position Requirements · Bachelor's degree or equivalent with 2-5 years experience in oncology, social work, nursing, community health education or related field preferred. Advanced knowledge of human behavior, social services, community organization, and healthcare systems preferred.Critical skills required include communication, patient service, assessment, teamwork, initiative, and versatility. Proficient in Microsoft Office, Lotus Notes and database management.
